It is a face that still talks. Almost nobody walks into a consultation asking to look different. They ask to look less tired. The gap between those two outcomes has very little to do with the product in the syringe. “Frozen” is the single most common fear patients bring into a Xeomin or Dysport consultation — usually stated within the first two minutes, usually apologetically. It is a reasonable fear. Most people have seen a result that reads as treated rather than rested, and almost nobody wants that. But frozen is not a side effect of a neuromodulator. It is a treatment-plan decision. Understanding which decisions produce it makes the whole thing far less of a gamble. The two we reach for first Xeomin and Dysport are the two neuromodulators we plan with most often at Balance Lab. They are not “lighter Botox.” They are different formulations of botulinum toxin type A, and each has a reason to sit at the top of the tray. 01 Xeomin The purified neuromodulator — manufactured without complexing proteins. A streamlined formula that many first-time and long-time patients prefer when the brief is “rested, not done.” 02 Dysport Fast onset and a softer, wider diffusion pattern. These five are not. 01 — Dose is a range, not a recipe The most common cause of an overdone result is a standardized dose applied to a non-standard face. Muscle strength varies enormously between people. Two patients with the same frown lines can need meaningfully different amounts of Xeomin or Dysport to reach the same result — and the one who needs less will look overtreated at the higher dose, every time. Body size doesn’t predict this. Age doesn’t reliably predict it either. It comes down to how strong a given muscle actually is, which is something an injector assesses by watching you move, not by reading a chart. What actually helps: Ask how your dose was determined and what would change it. A specific answer (“your glabellar complex is strong on the left, so we’ll weight that side”) is a better sign than a flat per-area price with no assessment attached. If a dose is too heavy, the only remedy is time — typically three to four months of it. That asymmetry is the entire argument for starting lower than the target and adjusting upward. Under-treating is a two-week inconvenience with an easy fix. Over-treating is a season. What actually helps: A two-week follow-up should be part of the plan from the beginning, not something you have to request. Neuromodulators typically begin working in three to seven days — Dysport often on the earlier side — and reach full effect around the two-week mark. That is the right moment to add a small amount if you want more, and far too late if too much was already placed. 03 — Treating the forehead in isolation This is the most common technical cause of a heavy, unnatural look, and it is worth understanding because it explains a result that confuses a lot of patients: my forehead is smooth but my brows feel low. The forehead muscle is the only muscle that lifts your brows. The muscles between and around them pull down. They exist in opposition. Treat the lifter aggressively without accounting for the depressors, and the downward pull wins — brows sit lower, the upper lids feel heavier, and the whole upper face reads flat. What actually helps: Horizontal forehead lines and frown lines are usually planned together rather than as separate items on a menu. Placement also matters as much as dose here — leaving the lower portion of the forehead muscle more active preserves the brow position that makes an expression look alive.
